On-Call Health Navigator (#642)

Position Summary:
The Health Navigator is a Medical Assistant who, under supervision from the RN Site Manager, also serves as a member of the chronic care management team collaborating with patients and their providers to optimize delivery of health care services to patients. The Health Navigator provides medical assistance and interpretation, checks out patients, facilitates referral and performs clerical tasks.

Essential Functions:
1. Provides highest quality of Medical Assistant care according to clinic policies and culturally competent care consistent with the agency’s Guide to Service Excellence and Cooperate Compliance Plan; safeguarding patient’s privacy rights and cooperates in resolution of patient complaints.

2.Takes vital signs, recognizing abnormalities and reporting them to nurses and providers.

3. Assist in optimizing patient flow.

Minimum Qualification:
Experience: Must be an experienced clinical Medical Assistant, having met the education and training requirements in the Medical Assistant field, and performing MA duties with a high level of proficiency and culturally compassionate care.
Preferred: Commitment to working in the Asian community, to community health care with familiarity with community health clinics. Medical Assisting Certificate
Certification, License, and Registration Required: CPR

Bilingual/bicultural in an Asian language: Cantonese, Vietnamese, Mongolian, Korean strongly desired.
