Position Summary:
The Health Navigator is a Medical Assistant who, under supervision from the RN Site Manager, also serves as a member of the chronic care management team collaborating with patients and their providers to optimize delivery of health care services to patients. The Health Navigator provides medical assistance and interpretation, checks out patients, facilitates referral and performs clerical tasks.
Essential Functions:
1. Interviews patient to obtain and record medical history using SOAP format in preparation for medical visits; obtains basic health information and relevant documents and records
2.Takes and records vital signs, recognizing abnormalities and reporting them to nurses and providers.
3. Performs basic nursing procedures such as injections (for immunization and tuberculosis skin test only), phlebotomy and dressing changes; performs basic in-clinic lab tests and obtains/processes specimens for lab send-out; performs tests such as electrocardiogram, spirometry, audiometry and Snellen test.
Minimum Qualification:
Experience: Must be an experienced clinical Medical Assistant, having met the education and training requirements in the Medical Assistant field, and performing MA duties with a high level of proficiency and culturally compassionate care.
Preferred: Commitment to working in the Asian community, to community health care with familiarity with community health clinics.
Bilingual/bicultural in an Asian language: Cantonese, Vietnamese, Korean strongly desired.
